Upload images of official documentation to verify your status as a Medical Professional. The ID.me team reviews document uploads within 24-72 hours at a secure U.S.-based facility
There are multiple ways of getting a copy of your paper documentation on to your computer in a digital format; including (but not limited to) using a scanner or taking a picture with a smartphone or tablet and e-mailing it to yourself, etc.
The documentation used to verify Medical Professional status depends upon the type of Medical Professional you are:
Physicians | Dentists
- Province or territory medical license
- Proof of board certification
The uploaded document must show:
- Full name
- Physician status
- License or certification number
Nurses
- Nurse Identification Card
- Letter of Employment
- Professional Certificate
The uploaded document must show:
- Full name (if on an ID Card: partial versions of full name, including initial for one of the names, are accepted)
- Active position as a nurse
- License or certification number
Document Requirements
The document uploaded must reflect the same name entered during the verification process. If documentation does contain current name (e.g.: maiden name is shown), additional documentation will be requested to demonstrate legality of name change – for example, a marriage certificate or legal name change document.
Note: We do not accept transcripts, department logos, business cards, emblems, or pictures of individuals dressed in uniform as a form of verification.
